Immunization Requirements

THE PREFERRED DOCUMENTATION ARE TITERS

The due date for all *documentation is Oct. 1 for summer/fall entry and for spring entry the due date is March 1

*Influenza vaccination due by Dec. 1

 

 

Measles, Mumps, Rubella (MMR): One of the following is required

  • Positive antibody titer (lab report OR physician verification of results required) OR
  • Two vaccinations

If series in process, submit where you are in the series and new alerts will be created for you to complete the series. If all three titers are negative or equivocal, and no documentation of two vaccinations has been documented, you must repeat the two-dose series. If you have two documented vaccinations and have a negative titer or equivocal titer, you must receive a one-time booster. If only one or two components of the MMR vaccination show lack of immunity, a one-time booster is recomended.

Varicella: One of the following is required

  • Positive antibody titer (lab report OR physician verification of results required) OR
  • Two vaccinations

If series in process, submit where you are in the series and new alerts will be created for you to complete the series. If the titer is negative or equivocal, and no documentation of two vaccinations has been documented, you must repeat the two-dose series. If you have two documented vaccinations and have a negative titer or equivocal titer, you must receive a one-time booster. History of Disease is NOT acceptable. 

Hepatitis B: (Not required for law students)

  • Three vaccinations AND a positive antibody titer (lab report or physician verification of results required) OR
  • A positive antibody titer alone will also be accepted (lab report or physician verification of results required). If you have documentation of three vaccines, please submit them along with your titer.

Tetanus:

  • Submit documentation of a Td OR a Tdap booster within the past 10 years. The renewal date will be set at 10 years

*Influenza: DUE annually BY Dec. 1 **Required for medical students.

  • Documentation of a flu shot administered during the current flu season (September-March) OR
  • Signed declination waiver. 

 

Tuberculosis (TB) screening

The due date for all *documentation is Oct. 1 for summer/fall entry and for spring entry the due date is March 1

 

Which screening is appropriate for me? The IGRA (blood test) or the PPD (skin test)?

Ask yourself these questions. If you answer YES to any of these questions you should provide an IGRA blood test which is either the Quantiferon TB Gold or the Tspot for your TB screening.

1.Have you received the BCG vaccine in the past?
2.Have you ever had a positive PPD skin test?
3.Have you ever had active OR latent TB?
4.Have you ever been treated for active OR latent TB?
5.Were you born outside of the US?
6.Have you lived outside of the US for >/= 1yr?
7.Are you currently taking immunosuppressants / steroids / anticancer medications?
8.Have you been vaccinated in the last 4-6 weeks for Measles, Mumps, Rubella or Varicella?

 

Students in dental*, post grad dental*, dental hygeine*, medicine, med tech, nursing, pharmacy, physical therapy, and social work*:

Are required to have ANNUAL TB screening tests.

Upon entry into your program, you are required to have one of the following based on the above screening questions: 

  • QuantiFERON Gold Blood Test OR T-Spot Blood Test (Preferred method) OR
  • Two TB skin tests (at least one-three weeks apart but no more than 12 months apart)
  • If positive result provide a clear chest X-ray result. Date has to be on/after date of positive TB screening result.

Renewal will be set for one year from date from skin test, blood test or chest xray.

Upon renewal, one of the following is required based on the above screening questions:

  • QuantiFERON Gold Blood Test OR T-Spot Blood Test OR
  • One-step test

Renewal will be set for one year from date from skin test, blood test, or TB Questionnaire.

*These programs have been updated to annual screening as of Fall 2018.

 

Students in genetic counseling, grad PhD, and law:

Upon entry into your program, you are required to have one of the following based on the above screening questions: 

  • QuantiFERON Gold Blood Test OR T-Spot Blood Test (Preferred method) OR
  • One-step PPD
  • If positive result provide a clear chest X-ray result. Date has to be on/after date of positive TB screening result.

No renewals are required at this time unless exposure occurs or required by clinicals.
