STEP 1

Go to the International SOS UMB portal.

STEP 2

In the upper lefthand corner, select destination countries in the Location List.

STEP 3

For each country you visit, find the city's risk rating and indicate it on the International Traveler Attestation Form with Risk Acknowledgement. If one of your destinations has a High Travel Risk or Extreme Travel Risk rating, your International Traveler Attestation Form with Risk Acknowledgement will be routed to International Operations for approval. Indicate the country's risk rating if your destination city is not listed.
