I would suggest starting with https://www.medicare.gov/, which has a chat feature, or the Administration for Community Living (https://acl.gov/help). According to CMS, in some cases, the local long-term care ombudsman may also have information on the home health agencies in a particular area and recommends visiting ltcombudsman.org, eldercare.acl.gov, or calling the eldercare locator at 1-800-677-1116. A very brief search makes me think that this person is not generally eligible for Medicare home health care if they are not homebound, among other criteria, but I think some local agencies might be more helpful.
