Please complete the following form to notify the UMB Office of the Fire Marshal of a fire code violation or fire safety concern. This form is for issues that do not require immediate response. If this is an emergency, call 911. If you witness an act that you believe will lead to a fire or cause injury, please call 410-706-3494. All other concerns will be investigated within 3 business days of form submission.

This form shall only be used to report fire safety issues at property owned by the University of Maryland, Baltimore. For concerns and issues with leased facilities, please contact the local fire safety authority having jurisdiction.
