Survey login instructions:
Non School of Medicine Faculty and Department Chairs:
Login using your email address and the password provided on the email message you received. If you did not receive a password on the email message, our records show you have an existing SOM network account. For assistance contact SOM help desk at 6-3998 or email help@som.umaryland.edu.
School of Medicine Faculty and Department Chairs:
Login using your email address and the password you have for either your SOM network password (SOM login, email or VPN password) or your BRAAN password if you do not receive network support from the SOM. For assistance contact SOM help desk at 6-3998 or email help@som.umaryland.edu.
MEDICINE and PHARMACY FACULTY and DEPARTMENT CHAIRS: The survey form has a radio button below your name and department that asks you to identify yourself as a department chair or faculty. The purpose of this selection is to generate an email to your department chair, if you are faculty, or to the dean or the dean's designee if you are a department chair for survey approval. If you are not a department chair but select the department chair option, your survey will be sent to your dean instead of your department chair.
If you are FACULTY, but not a department chair, select the SECOND Radio button for FACULTY.
If you are a DEPARTMENT CHAIR filling out the survey, select the FIRST radio button for DEPARTMENT CHAIR.
LAW and SOCIAL WORK FACULTY do not need to select the FACULTY or DEPARTMENT CHAIR radio button. This option is greyed out on the survey form.
As a department chair or dean, you have the option to print out submitted surveys individually for review before approving them.
