ASC Help Desk Forms Adobe Acrobat is needed to view these files. If you need Acrobat, click on Get Acrobat Reader. The ASC Help Desk can reset passwords for Enterprise (campus-wide) applications. Click here for a list of applications and services supported by the ASC Help Desk. Users can visit the ASC Help Desk during business hours to have their password reset. Please bring photo ID for verification. If users have access to a University ID and a scanner or fax machine, users can use the Password Change Request form to request a password reset remotely. Users will need to print out and complete and sign the Password Change request form. - Print out, complete, and sign the Password Change Request form.
- Return the form with a copy of a Photo ID to the ASC Help Desk.
- Users can fax the completed form and a copy of their photo ID to the ASC Help Desk to 410-706-4191.

NOTE: The ASC Help Desk cannot process faxed or emailed Password Change Requests without a copy of a Photo ID for identity verification. Please include a valid phone number on the Password Change Request form. The ASC Help Desk will call you at this number once we have processed your request and reset your password.
