CONTACT INFORMATION

I AGREE TO THE FOLLOWING STATEMENTS:

1. At the beginning of the semester, I will meet with ESDS staff to discuss policies, procedures and responsibilities of being a notetaker.*
2. I will meet with Celeste Gerhart to fill out payroll paperwork. I will provide an original Social Security Card. If I don’t have a card, I know I will have to go to the Social Security Office to receive a card in order to be paid by the University.*
3. I will provide the ESDS Office with electronic timesheets during the appropriate pay periods (2 weeks schedule). I will provide the office with a copy of my notes to confirm my work for the University.*
4. I will take notes as I normally do in my classes and will either make a copy of them for the student via photocopy for handwritten notes or through email attachment. Notes will be sent to the student directly or to the ESDS if the student chooses to not disclose themselves. The ESDS office would then forward on the notes to the student.*
5. I understand that notes must be forwarded no later than 48 hours after class ends. The notes must be saved as 97-2003 versions of Microsoft Word document or included in the notes section of a PowerPoint presentation.*
6. I understand that if the student is absent from class and does not inform me or ESDS about their absence, that I should not forward my notes to the student. (Notes can be forwarded if the absence is disability related).*
7. I understand that if I am unable to attend class that I must contact ESDS or the student. If I am able to find a substitute note-taker, I understand that the substitute note-taker will not be paid.*
8. I understand that if at any time I am unable to continue providing the note-taking service that I must inform ESDS as well as the student (if applicable), so that a replacement notetaker can be assigned.*
